Chanelle Fokum, RN, BSN, BIT, MBA
Co-Founder & COO
Chanelle is a registered nurse with clinical and administrative experience. She brings a unique blend of healthcare knowledge, compassion, and leadership to HC Home Care.
She ensures the highest standards of care are delivered while building meaningful relationships with individuals, families, and care teams.
